Case Management Services
•SPECIAL DELIVERY CASE MANAGEMENT is a free service that exists to promote
the health of pregnant women and their infants through education and resource referrals on a person to person basis.
We inform our clients in order to empower them to make the best choices for health.
Admission to this program is limited to Medical Assistance eligible Frederick County pregnant women
who have one or more risk factors as identified by their Obstetric (OB) provider.
Women enrolled in this program are phone case managed, will meet their RN case
manager at least once during the pregnancy and can expect a post partum home visit
after giving birth.

HE MARYLAND POSTPARTUM INFANT AND MATERNAL REFERRAL SERVICE
is a program where local hospitals communicate with the MCH program if a Frederick
County woman is identified as needing case management or care coordination
assistance after she's giving birth.
