LEAD POISONING CASE MANAGEMENT
•Exists to assist families of children identified with an elevated lead level of 15µg/dl or
higher
•Patient education to decrease or eliminate exposure is conducted
•Provides a home visit with a nurse and a regional sanitarian
•Issues an official notice of elevated blood lead level to the landlord if the home is a
rental property.
•A Lead Assessment and care coordination is conducted for lead levels of 10-14 µg/dl.
LEAD POISON PREVENTION AND EDUCATION is provided to parents, tenants and property owners about:
•Environmental Control Measures
•Lead awareness and prevention
•Landlord and tenant rights and responsibilities
•Screening opportunities
