What Makes Citizens Special?
Founded in 1976 by the Board of County Commissioners as a model of human caring for the residents of Frederick County, Citizens Care and Rehabilitation Center has consistently exceeded its goals related to resident care and community outreach.

Mission
Our mission has grown over the years to include not only long-term care, but short-term rehabilitation as well. We continue to provide excellent care, every time!

The Board of County Commissioners, Citizens/Montevue Board of Trustees, and County Government leaders have supported our efforts by approving the construction of our new, state-of-the-art facility for both Citizens (skilled nursing and rehabilitation) and Montevue Assisted Living.

To meet the changing needs of our residents, Citizens offers the following services on a daily basis in our existing building:
  • Skilled Nursing Care 24-hours a day, every day.
  • Skilled Rehabilitation Care, whether for a few days, a few weeks, or during custodial care.
  • Wound care, including that for diabetic ulcers, pressure wounds, and those wounds resulting from accidents or illnesses.
  • Nutritious meals and snacks throughout the day, on a rotating schedule. Resident Food Committee meets monthly to choose festive celebratory meals and holiday celebrations.
  • Activities for all ability levels, including praise & worship, bingo, arts & crafts, movies/music themed events, and even "The Gathering," a special time & place for residents with specific cognitive needs to socialize and have lunch together.

The All New Citizens Care and Rehabilitation Center Features
  • 170 graciously appointed resident rooms, each individually climate-controlled with private window and window seat, built-in closet and cabinets, state-of-the-art beds, & wall-mounted flat screen televisions with cable access.
  • 74 residents will reside in all-private accommodations with private baths
  • 96 residents will have a private living space and share a private bathroom.
  • Simulated wood flooring with inlaid designs.
  • Ample living spaces, featuring fireplaces and country kitchens in each resident-centered neighborhood.
  • Boutique-style gift shop staffed by our Volunteer Auxiliary.
  • Tastefully appointed dining rooms for seated service, featuring specially-crafted menus.
  • Interdenominational Chapel for quiet devotion or meditation.
  • Video surveillance throughout campus, inside and out, for added security.
  • Dental/Podiatry Suite

Amenities
  • Spa-like bathing areas with whirlpool tubs, spacious showers, and heated towel racks
  • Specially designed rehabilitation Courtyard.
  • Oxygen piped into all rooms for maximum convenience and service.
  • Ceiling-mounted lift system in designated rooms to allow for dignified transfer of residents in and out of bed.
  • “Stay rooms” so that families who wish to remain at Citizens during a loved one’s health crisis can get some much-needed rest in comfort.
  • “Snoezelan”, or multisensory room, will provide those residents with dementia a space in which to relax and get away from the sights and sounds of the “real world.”
  • Multiple public and private gathering rooms for residents, families, and the community-at-large.
  • Several landscaped outdoor courtyards with water features, gardens,pathways, and seating areas to allow visits outside in nice weather.
