How to Apply
- Print an MCHP Application Form

- Call toll-free at 1-800-456-8900 and ask for a MCHP application form, or
- Apply online at www.marylandsail.org
- Make sure to put your phone number on your form so someone can contact you if they have questions.
- Mail in your completed application form to:

Information Needed to Apply
- Proof of US citizenship or residence (Birth certificate; passport; naturalization papers; affidavits)
- State-issued photo identity card (Driver's license or MVA ID)
- Sources & amount of income
- Names, birth dates, social security numbers of family members (The guardians and their dependants)
- List other health insurance coverage
Once You Have Been Approved
